John Cutler you're missing "Pay Down Tech Debt" in "Do This".

John - this is brilliant. What do you think is behind this tendency? My hypothesis: people veer towards silver bullets that they think will solve all their problems. Unfortunately, sustainable solutions that require more work than that. Every new tool or dependency you add increases complexity if not handled correctly.

Any suggestions to get out of the "Defect Fixing Mess"?

Yeah, the daily juggling exercise. Capacity allocation, explore vs exploit, enabling work vs business value, lean flow, pull, prioritization, etc. Care for the garden is a good metaphor for it. Full transparency on all activities, avoidance of hidden work and proper lean portfolio management are key. Christian Gary Yannick

Agree and have experienced this first hand multiple times. The “enhance existing capabilities” needs to address poorly thought out or executed functionality especially when customer facing (and this must include external customers as well as your implementation, training and support teams.)

The challenge is, John Cutler, for legacy enterprises squarely stuck in the paradigm on the right, how to start realigning people against *client outcomes* that will require a mix of Innovation, Iteration, and Support. The mindsets & culture that got them to this point won’t help them get to the next level.

been shot in that "Not This" movie before.....

Oddly this is a lesson that is often not learned the first time. Eventually that critical period will lead to a plateau and the choice will be made again to go with that second column of priorities. Sustainability is not an OKR.
